Part Time Weekday and Weekend Hours available Per Diem

Constant Care Health Services is currently seeking highly trained Physical Therapists (PT) for our 5 Star Skilled Nursing Facilities throughout the Bronx. Our facilities are committed to providing high quality services and to do so we need amazing Physical Therapists (PT). We provide ample training, support and communication to our team of healthcare professionals to ensure their success!

Qualifications for Physical Therapist (PT):

  • NYS PT License

  • Strong Communication Skills

  • Must be able to work weekends when needed

Responsibilities for Physical Therapist (PT) includes:

  • Assessing the client needs on admission for service as appropriate.

  • Implement a medical plan of care and revising as necessary.

  • Initiating appropriate preventive therapeutic and rehabilitative techniques.

  • Educating the client and family regarding treatment modalities and use of equipment and devices.

  • Providing consultation to other health care professionals.

  • Communicate with the physician and other health care professionals regarding changes in the client’s needs.

  • Attends and participates in case conferences, as indicated, to share and/or gain knowledge, especially when services are provided to special populations. Such knowledge supports clinical application of physical therapy in special populations to recognize symptoms and improve case management techniques specific to these populations.

  • Participates in quarterly peer review that is documented and signed by both parties
