Position: Home Health Aide (HHA/CNA)
Location: Based in Orange Park, FL.
What You'll Do:
As a Home Health Aide (HHA/CNA), you'll be a vital part of the care team-supporting patients with personal care needs while promoting dignity and independence.
Key responsibilities include:
  • Assisting with personal care, bathing, and hands-on care in accordance with the patient's plan of care
  • Accurately and promptly documenting each visit per agency policy
  • Notifying the supervising RN of any changes in the patient's condition
**Note: This role involves multiple short visits throughout the day-not shift work or extended 12-hour cases.
Qualifications:
  • Completion of a Home Health Aide training program and competency evaluation meeting Medicare's Conditions of Participation
  • Valid driver's license, reliable transportation, and auto insurance (required)
  • Ability to travel to patients' homes throughout the day
  • Previous home health experience is a plus, but not required
