Key Responsibilities
  • Conduct comprehensive assessments to understand client needs and develop individualized treatment plans that reflect these needs.
  • Deliver therapeutic counseling sessions via telehealth platforms, employing a variety of therapeutic modalities to support clients in reaching their goals.
  • Ensure timely medication management by providing medication prompts and reminders, assisting clients with medication adherence, and monitoring refill needs.
  • Assist clients in navigating public transportation and coordinating transportation services for essential medical appointments.
  • Monitor and report on clientsโ€™ general mood and safety, ensuring compliance with agency safety protocols and positively influencing client behaviors.
  • Facilitate life skill workshops aimed at helping clients with daily living activities such as meal planning, grocery shopping, and cooking.
  • Maintain meticulous and timely documentation of all client interactions, therapy notes, and administrative tasks to comply with legal and regulatory standards.
  • Collaborate with multidisciplinary teams, providing timely updates and insights to ensure coordinated care efforts.
Patient Population / Client Focus

You will work with a richly diverse group of clients, including individuals from various cultural backgrounds. Qualifications
  • A Masterโ€™s degree in counseling, social work, psychology, or a closely related field is required.
  • Licensure as an LCSW, LPC, LCPC, LMHC, or LMFT in California is mandatory. Please note that we currently cannot accept applicants with an associate license, as they are not billable through Medicare.
  • Proficient computer skills, especially with Microsoft Office Suite, are essential for documentation and client management tasks.
  • Strong ability to work autonomously, demonstrate initiative, and adapt in a fast-paced environment handling multiple client cases.
Preferred Experience
  • Experience working with geriatric populations is a plus, though not required.
  • Familiarity with telehealth practices and remote counseling technologies is preferred as it will aid in seamless client interactions.
