Work Environment

This is a remote position, designed to offer flexibility while ensuring that you remain connected to the clients you serve. Our workweek typically runs from Monday to Friday, 8:00 AM to 4:30 PM, with occasional requirements for evening or weekend hours based on client needs. Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs to inform individualized care plans that address emotional, mental, and social factors affecting their well-being.
  • Collaborate closely with clients, their families, and interdisciplinary teams to develop and implement tailored care strategies that align with the seniors' preferences and goals.
