Gotham Enterprises Ltd is seeking a compassionate and dedicated Licensed Clinical Social Worker (LCSW) to provide vital support to seniors and their families in a remote setting. This full-time position focuses on enhancing the quality of life for older adults within our community, utilizing your social work expertise to assess their psychosocial needs and coordinating access to necessary services.

This is a remote position, designed to offer flexibility while ensuring that you remain connected to the clients you serve. Our workweek typically runs from Monday to Friday, 8:00 AM to 4:30 PM, with occasional requirements for evening or weekend hours based on client needs. You will have the opportunity to work with a diverse team of healthcare professionals, all committed to providing holistic care to seniors.

• Conduct comprehensive assessments of seniors' psychosocial needs to inform individualized care plans that address emotional, mental, and social factors affecting their well-being.
• Collaborate closely with clients, their families, and interdisciplinary teams to develop and implement tailored care strategies that align with the seniors' preferences and goals.
• Provide counseling and emotional support to clients and families, assisting them in navigating transitions such as moving into assisted living, coping with loss, or dealing with chronic illness.
• Advocate for clients' rights, ensuring they have access to necessary community resources, services, and support networks.
• Monitor clients' conditions through regular visits, evaluating living circumstances, and addressing any concerns regarding safety and comfort.
• Facilitate family meetings to enhance communication, resolve conflicts, and ensure a cohesive approach to the senior's care.
• Educate clients and families about available resources, support groups, and programs that promote social engagement and lifestyle enrichment.
• Maintain accurate documentation within electronic health record (EHR) systems, tracking assessments, care plans, and progress notes while adhering to compliance standards.
• Stay updated on best practices in social work, ethics, and policies to ensure high-quality care delivery.

This role primarily serves the senior population, supporting individuals who may be experiencing varying degrees of physical and mental health challenges. You will work closely with clients who may require guidance in adjusting to life changes, managing chronic conditions, or accessing community services. Your efforts will directly contribute to enhancing their quality of life and overall well-being.

• A Bachelor's degree in Social Work from an accredited institution is required.
• Active and valid state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of experience in a social work capacity, ideally within a community center or geriatric care environment.
• Strong foundation in social work principles, knowledge of aging-related issues, and familiarity with relevant community resources.
• Skilled in assessment, counseling, and crisis intervention techniques.
• Exceptional interpersonal skills, with the ability to build rapport with clients, families, and professionals effectively.
• Proficient in using electronic health record (EHR) systems and adept with various computer applications.

• Experience working in hospice or end-of-life care settings is a plus.
• Familiarity with local community organizations and resources specific to the Newington, CT area.
• Advanced knowledge of geriatric social work practices and interdisciplinary collaboration.
