Potential Opportunity: Internal Medicine Physician (Board Certified)
Location: Tyler Centennial VA Clinic – Tyler, Texas
Contract Type: Full-Time - Contract
Hours: Monday – Friday, 8:00 AM – 4:30 PM (No Federal Holidays)
Position Overview
We are seeking a Board-Certified Internal Medicine Physician to provide high-quality primary care services to our nation’s Veterans at the Tyler Centennial VA Clinic. As a key member of a Patient Aligned Care Team (PACT), you will manage a longitudinal panel of Veteran patients, focusing on disease prevention, chronic disease management, and acute care.
Key Responsibilities
- Patient Care: Manage a PACT panel of approximately 1,100 to 1,300 Veterans, seeing 9–12 patients per day.
- Clinical Duties: Perform history and physical exams, medication reconciliation, and evaluation of diagnostic tests.
- Chronic Disease Management: Treat conditions common in the Veteran population, including DM, COPD, CAD, CHF, and hypertension.
- Tele-Health: Utilize VA Video Connect for home visits and integrated care.
- Collaboration: Provide leadership within the PACT team and supervise Health Professions Trainees (HPTs) when rotating through the service.
- Documentation: Maintain 100% accurate and timely electronic health records (EHR) in accordance with VA privacy and professional standards.
Required Qualifications
- Education: Completion of a two-year residency in Internal Medicine or Family Practice.
- Certification: Board Certified or Board Eligible in Internal Medicine or Family Medicine (ABMS or AOA) within 4 years of training.
- Licensure: Current, full, and unrestricted license to practice medicine in any U.S. State, Territory, or the District of Columbia.
- Certifications: Active Basic Life Support (BLS) certification.
