Job description

Are you a compassionate healthcare professional with a heart for serving seniors?
If so, Seniority Healthcare invites you to join our team as a Per Diem COMPASS Nurse in the Exton area. COMPASS is an acronym for Collaborative Outpatient Management for Palliative and Aging Support Services. This unique role provides in-home nursing and case management services to our patients, offering you the opportunity to make a significant impact in the lives of our geriatric patients. This part-time role.
Responsibilities:

  • Communicate effectively with patients, families, providers, and community staff regarding all aspects of patient care.
  • Provide updates on treatment plans, address concerns or changes in the patient's condition, and ensure that everyone involved in the patient's care is informed and involved in the decision-making process.
  • Manage and coordinate care for patients, including documenting and assisting with orders for testing, medications, and referrals.
  • Upload and document all health information into the patient's chart accurately and timely.
  • Maintain accurate and up-to-date records of all patient interactions, treatments, and outcomes.
  • Obtain vital signs and assist providers as needed during patient visits.
  • Provide hands-on care to patients, including assisting with examinations and providing support and comfort to patients and their families.
  • Conduct medication reconciliation and maintain a professional relationship with the PCP. Work closely with the patient's primary care provider to ensure that medications are managed safely and effectively and that any changes in medication are communicated promptly.
  • Coordinate referrals, therapy orders, and telemedicine appointments. Ensure that patients receive the services they need in a timely manner.
  • Alert providers of acute issues. Monitor the patient's condition closely and alert providers to any changes or concerns that may arise.
  • Check for patient updates and communicate with case management departments. Stay informed about the patient's status and treatment plan, and communicate with other healthcare providers to ensure continuity of care.
  • Maintain HIPAA compliance and communicate effectively with other members of the care team.
Qualifications:
  • Minimum of 4 years of experience in a similar role, working with geriatric populations.
  • LPN or RN license.
  • Proficiency in EHR systems.
  • Phlebotomy experience needed.
  • Strong communication skills and ability to work effectively in a team environment and remotely.
  • In-depth knowledge of health care services and medical terminology preferred.
