Job description

Description

This position provides quality nursing oversight, linkage of residents to primary care providers and/or mental health services, and monitors medication compliance of residents on a daily basis.

Responsibilities

  • Implements medication policies and procedures and ensures compliance with OASAS 820 Regulations.
  • Initiates and coordinates initial referrals with primary medical doctors and mental health providers and assists when necessary.
  • Assists with follow-up phone calls and advocacy on the resident's behalf when necessary.
  • Maintains knowledge of the individualized care plan for residents and provides support to the residents according to their care plan.
  • Collaborates with other clinical and medical staff members in advocating for residents during Medicaid managed care and insurance company reviews.
  • Accurately completes medical documentation for each resident.
  • Provides basic First Aid and CPR when necessary and maintains current certifications.

Requirements

  • One year's experience practicing as a Licensed Practical Nurse.
  • Requires an unrestricted New York State License or Certification: Licensed Practical Nurse, CPR and First Aid Certified.
  • Must possess a valid New York State driver's license.
