Downtown Counseling Services is actively seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our compassionate team. This full-time position offers the unique opportunity to work remotely while providing crucial support to seniors and their families in a hospice center environment. You will leverage your clinical skills and understanding of psychosocial dynamics to enhance the quality of life for those we serve, ensuring they receive the dignity and care they deserve.

Work Environment

This position is entirely remote, allowing you the flexibility to perform your duties from the comfort of your own home while still being an integral part of a collaborative team focused on enhancing senior care. While your core schedule will typically run from Monday to Friday, 8:00 AM to 4:30 PM, occasional evening or weekend hours may be necessary to best accommodate client needs. A strong emphasis on communication and collaboration with fellow team members will be vital, and flexibility in your work schedule will be paramount to ensure continuity of care.

Key Responsibilities
  • Conduct in-depth assessments of seniors' psychosocial needs, which encompass emotional, mental, and social determinants of health, establishing a foundational understanding of each client's situation.
  • Develop personalized care plans by collaborating with clients and their families, ensuring the inclusion of their goals, preferences, and values, thereby promoting active participation in their care.
  • Provide one-on-one counseling and emotional support to clients, helping them navigate through transitions such as medical changes, loss, and other significant life events.
  • Advocate for clients’ rights and access to necessary community resources, acting as a liaison to ensure they receive appropriate services and support tailored to their individual challenges.
  • Document all client interactions, assessments, and care plans meticulously, ensuring compliance with state regulations and organizational standards.
Patient Population / Client Focus

The primary focus of this role will be seniors residing in hospice care settings. You will work with individuals in varying stages of their lives, addressing sensitive issues related to end-of-life planning, mental health crises, and emotional distress. A large part of your work will involve engaging with the families of these seniors, providing them with the tools and support necessary to manage their loved one's care and emotional needs. Your expertise will guide families through difficult transitions while fostering a supportive environment.

Qualifications
  • A Master’s degree in Social Work from an accredited institution is required, reflecting your thorough understanding of social work theory and practices.
  • A current and valid state licensure as a Licensed Clinical Social Worker (LCSW) is essential to ensure compliance with state regulations.
  • A minimum of two years of relevant experience in a social work capacity, ideally within a geriatric or hospice setting, providing you with the necessary background to excel in this role.
  • Profound knowledge of social work principles, aging-related issues, and available community resources to effectively guide clients.
