Your Opportunity:

he RN Case Manager works as a member of the health care team, collaborating on the delivery of holistic, comprehensive care to meet the needs of individuals, families and/or communities within their level of competency. The RN Case Manager provides leadership in nursing assessment, planning, implementation and evaluation of care for the purpose of promoting, maintaining or restoring health, preventing illness, injury and disability or supporting peaceful death. The RN Case Manager provides care throughout the life span, in all stages of health and complexities of illness. The RN Case Manager is accountable to the public for his/her own practice, which is monitored and regulated (Health Professions Act, 2001). The RN Case Manager is responsible for ensuring that practice meets legislative requirements and the practice standards of the College and Association of Registered Nurses (2008). The RN Case Managers are guided by their level of education, expertise, experience, demonstrated skill level, Alberta Health Sciences (AHS) - Calgary zone policies and job description.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:

Additional Required Qualifications:

Preferred Qualifications:

Baccalaureate Degree in Nursing is preferred, however, relevant experience will be considered. Three years of recent relevant experience in community health setting as a Care Coordinator (obtained within the last five years). Previous community and geriatric related experience. Excellent organizational, oral and written community skills; high level of independence & decision-making ability, as well as integrity for working with confidential information.
