Description:

The Community Programs Manager will oversee the Community Services Department and prgrams within an integrated care team model. This position will develop and maintain programs that support Hurtt's health services through community and patient engagement, outreach, enrollment to public benefits, education, patient navigation, community linkages, and social determinants of health services. This will include working with community partners and local and state organizations. Requires a highly organized individual with initiative to pursue program improvements and emerging community health initiative to best serve our patients and community. The goal is to improve access, quality, health outcomes, and patient satisfaction by addressing the social drivers of health care.


This is a full time position located on-site in Tustin, CA.

What You'll Do:

Job Responsibilities & Duties

  • Supervise, train, and develop all personnel in Outreach, Health Education, Patient Navigation, Enhanced Case Management, and Enrollment in the Community and Patient Services departments according to rules and regulations set by the administration.
  • Ensure patient confidentiality and knowledge of HIPPA.
  • Determine and implement workflows, integrate programs within HFHC, and develop written procedures that are efficient and effective.
  • Assist in the preparation of monthly departmental and project reports.
  • Participate as a member of the management team in the overall management of the health center sites.
  • Ensure all operations meet departmental budget guidelines.
  • Manage risks by ensuring that all services provided meets quality standards determined by internal and external regulations/guidelines. Uphold core values and strategic goals of the organization.
  • Develop new policies and recommend changes of existing policies regarding community and patient services to improve efficiencies and effectiveness.
  • Monitor specific aspects of statistical data.
  • Brief the director/management teams on relevant matters.
  • Assist patients with community linkages and benefits.
  • Track and follow-up on tasks and projects through completion.
  • Assist patients with social determinant of health barriers such as transportation, housing, employment, childcare, etc.
  • Develop community partnerships and linkages to educate community members on HFHC services and to develop referrals system to better support patients.
  • Outreach to community members to link them to services at HFHC, including developing internal and external outreach and marketing material
  • In-reach and integrate with Hurtt programs and providers to increase awareness of Hurtt programs and community linkages.
  • Learn and become familiar with what community supports are available to our patients (food, basic needs, transportation, insurance, etc.).
  • Develop group educational and support programs as assigned.
  • Assist patients with resolution of issues—navigating governmental departments, etc.
  • Assist medical staff in researching options for the patients (other than medical referrals and medications).
  • Coordinate health fairs and other community events and assist with event coordinating and fundraising.
  • Assist with bringing new resources to the community.
  • Assist with advertising promotions.
  • Collaborate with community partners.
  • Attend community partner and collaborative meetings as appropriate for the department.
  • Provide presentations to the community regarding HFHC and outreach services.
  • Attend community partner and collaborative meetings.
  • Work with the Hurtt Grant Writer and Finance department to develop grant applications, program implementation, grant reports, grant compliance, etc.
  • Participate in QUIC Committee and QI initiatives
  • Represent HFHC in a professional and compassionate manner and reflect the value of HFHC.
  • Perform other duties as assigned.

Minimum Qualifications

  • Master in Public Health degree preferred. Master's degree in similar fields preferred but not required. Bachelor's degree in communications, business administration, grant administration, or similar field required.
  • Demonstrated knowledge of social service program development and implementation.
  • Comprehensive experience in researching and identifying funding opportunities, analyzing diverse data, and possessing the ability to work collaboratively and independently to achieve stated goals.
  • Must be able to work beyond normal working hours, including weekends.
