Full-Time Nashua, NH
Greater Nashua Mental Health is seeking a Community Support Specialist to provide community-based support to individuals working toward recovery from mental health and co-occurring conditions. This role focuses on building engagement, supporting independence, and helping clients improve functioning within their natural environments through individualized, strengths-based interventions.
Key Responsibilities
  • Provide direct, community-based support services to individuals in their homes and community settings.
  • Engage clients using evidence-based interventions such as Motivational Interviewing.
  • Maintain a caseload and meet productivity expectations for direct client service hours.
  • Conduct outreach to re-engage clients who have disengaged from treatment.
  • Complete accurate and timely documentation for all services and outreach activities.
  • Collaborate with multidisciplinary teams to support coordinated, client-centered care.
  • Participate in team meetings, supervision, and ongoing training activities.
  • Maintain schedules and document services in the Electronic Medical Record (EMR).
  • Support clients in building life skills, accessing resources, and achieving recovery goals.
  • Complete mileage and expense reporting in a timely manner.
  • Perform additional duties or special projects as assigned.

Qualifications
  • Associate's degree in Psychology or related field required; Bachelor's degree preferred.
  • 2+ years of experience in human services or behavioral health field.
  • Knowledge of mental health conditions and recovery-oriented practices.
  • Strong communication, interpersonal, and engagement skills.
  • Ability to work independently and collaboratively within a team environment.
  • Strong organizational and time management skills.
  • Proficiency with Microsoft Office and electronic documentation systems.
  • Ability to exercise sound judgment and maintain professional boundaries.
