Job description

Job Title: Community Health Worker

Location: Community Medical Center

Department Name: RWJBH Community Health Project

Shift: Evening

Pay Range: $21.69 - $30.64 per hour

Pay Transparency:

The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Please note the physical location is our Community Medical Center.

Job Overview:

The Outreach Community Health Worker will primarily maintaining a presence in the community with patients to promote, motivate and improve healthy living.The CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Qualifications:

Required:

  • High School Diploma or equivalent
  • Current and Valid Drivers License

Preferred:

  • Bi-Lingual in English and Spanish 

Scheduling Requirements:

  • Full time
  • Evening Shift (01:00pm-09:00pm)
  • One Saturday shfit per mpnth, one holiday (09:00am-05:00pm) 

Essential Functions:

  • Meet patients in a clinic, hospital, ER, and where patients are located and conduct needs assessment using Social Determinants of Health Tool
  • Schedule weekly follow-up calls and home visits to patients
  • Help patients with social issues like homelessness, substance abuse, hunger, transportation, and many other social issues that are impacting the members lives
  • Assist patients with organizing their records, making follow-up appointments with a Primary Care Physicians or Specialists
  • Help patients with the tasks of getting medical care, and work on health goals by helping to plan healthy meals or finding time to exercise
  • Help patients access resources, including quality of care, childcare services, food, housing, transportation
  • Maintain relationship with the Community Based Organization
  • Able to travel and meet the members where they at; may include local Churches, homeless shelter, and members residence
