Community Health Navigator Jobs in Nebraska (NOW HIRING)

The Community Health Navigator (Navigator) is an essential member of the Institute for Population Health (IPH) North Omaha community engagement team. They support community members with navigating the healthcare and social services systems. The navigator provides orientation, referrals, and ongoing support to individuals and families in North Omaha experiencing unmet health and social needs. Partnering with health systems, clinics, and other community-serving organizations, the navigator works with community members to build a supportive network for the improvement of their overall health. Additionally, the navigator conducts community outreach, builds relationships with key community stakeholders, and represents the IPH at community-based events. The Community Health Navigator also provides basic screenings and health education, and makes referrals for ongoing healthcare and disease management as appropriate.

• This position requires a high school diploma or equivalent. 
• Applicants should have 1-3 years' experience in health promotion, social services navigation, or benefits enrollment. 
• Must be very familiar with the North Omaha geographic service area, with proven positive relationships with community agencies and populations. 
• Comfort in adapting to needs and expectations of a wide range of stakeholders from the university and the community.  
• Effective communication skills with diverse stakeholders.

Additional qualifications that are strongly preferred:

• Community Health Worker (CHW) certification
• Bilingual (Karen, Spanish, Arabic, Somali, or other population languages)
• Experience building trusted relationships in diverse communities, and with community-based organizations
• Has a deep understanding of community needs and existing health disparities locally and nationally in order to partner in developing sustainable solutions.

Licenses/Certifications:

• Community Health Worker (CHW) certification preferred. Other position-specific trainings may be required after hire (e.g., blood pressure screening, health insurance enrollment, etc.).

• Support community members with resource navigation, provide referrals, and follow-up as individual participant needs require.
  • Support program participants with healthcare navigation and advocacy including but not limited to: appointment scheduling, navigating social barriers to healthcare (e.g., transportation, childcare), building health literacy, and communicating any barriers to providers and support teams.
  • Support program participants with non-medical resource navigation and advocacy including but not limited to social needs related to: housing, food, parenting, employment, finance, justice, immigration, education, and language.
  • Provide application, enrollment, and navigation support for public benefits and eligibility-related programming.
  • Maintain up to date records of participant support efforts, submitting relevant progress notes and data into relevant tracking systems according to IPH policies and procedures and professional best practice.
• Provide chronic disease management support.
  • Per area specialization and appropriate credentialing, provide screenings, disease management education, and referrals.
  • Collaborate with providers, specialists, and community-based health organizations to address short- and long-term disease management goals.
  • Support patients with ongoing disease management strategies.
• Develop and maintain strong partnerships with community-based organizations, neighborhood groups, churches, schools, and other entities that support the community.
  • Represent the IPH at select community-based events and activities, as determined in collaboration with IPH leaders.
  • Conduct community outreach to promote Creighton, IPH, and the Highlander and any related efforts.
• Support the daily operations of Creighton at Highlander space.
  • Maintain a clean and welcoming physical space.
  • Greet and welcome visitors who come into the Highlander space.
  • Ensure all materials are properly stored and secure.
  • Work with Creighton student volunteers, including training and role modeling the characteristics of culturally sensitive services.
  • Contribute to development and maintenance of the navigator data intake structure, to support quality assurance and improvement efforts.
• Additional activities in support of the IPH as requested, within the community and at the university.
