Location: Various locations across Alberta Community Health Workers (CHWs) serve as liaisons between healthcare providers and the community, promoting access to care and health education. Key Responsibilities: Assist individuals in navigating healthcare services and resources. Provide education on health topics and disease prevention.
Conduct outreach and support community health initiatives. Collect data on community health needs and barriers to access. Advocate for patients and connect them with necessary services.
Requirements High school diploma; relevant certification or degree preferred. Experience in community outreach or healthcare settings. Strong communication and interpersonal skills.
