In a region renowned for abundant sunshine and beautiful year-round weather, this Veterans Home of California is just minutes away from the scenic Pacific Ocean, historical Downtown Los Angeles, legendary Hollywood and beautiful San Fernando Valley. Completed in September 2010, the gorgeous Veterans Home is a 3960-bed, state-of-the-art, long-term healthcare facility for California's Veterans. Adjacent to the Memory Care unit is a beautiful, open-air garden with cozy seating area where Veteran residents can enjoy being outdoors in a safe and comfortable environment. The Home offers an 84-bed Residential Care Facility (assisted living) for the Elderly, a 252-bed Skilled Nursing Facility and a 60-bed Memory Care unit. Physical Therapist Responsibilities
  • Perform care consistent with the scope of licensure and in accordance with the CalVet Home policies and procedures and consistent with generally accepted Physical Therapy practices. Care provided must be sufficiently complex and consistent with the unique skills of a physical therapist.
  • Provide Physical Therapy services pursuant to a consultation prescribed by the resident's physician.
  • Provide an initial assessment/evaluation on each referral, which includes nature and extend of impairment, develop a treatment plan consistent with those findings, and be responsible for communicating the treatment plan to the Resident and any relevant medical personnel.
  • Complete reports or assessments/evaluations and progress notes utilizing CalVet approved documentation forms in the Resident's chart.
  • Maintain documentation on all residents referred for services. Records are to be kept in accordance with CalVet – WLA policies, Department of Public Health, USDVA, and Medicare Standards.
PT Qualifications
  • Shall have a minimum of 1 year experience within the last 3 years performing services similar in the above listed job description herein, in a public or private institution.
  • Education: Graduation from an approved Physical therapy program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE).
  • Current and valid license issued by the Federation of State Boards of Physical Therapy (FSBPT), and a current and valid license issued by the Physical Therapy Board of California in order to practice Physical Therapy in the State of California.
  • Maintain current CPR and BLS certification.
