Duties & Responsibilities
Collaborative Drug Therapy Management Pharmacist: New York City Health and Hospitals Corporation (Bronx, NY) seeks Collaborative Drug Therapy Management Pharmacist to serve as a liaison between the patient, ambulatory, clinical and pharmacy teams, leading to efficient provision of drug therapy and positive patient outcomes. Provide consulting services to patients regarding the effective use of medications, importance of adherence, and awareness of drug interactions. Partner with the Office of Ambulatory Care and collaborating physicians to provide patient care. Provide other preventative healthcare services such as immunizations. Serve in an essential role in disease state management, identifying ways to optimize medication therapy and ensuring optimal patient experience. Utilize facility and enterprise formularies, including restricted drugs and required drug request documentation, as well as the prior authorization process for insured patients. Collaborate with other providers in the clinic and with the pharmacy team to establish an efficient workflow. Responsible for drug management therapy of patients with specified disease states in an ambulatory clinic setting. Adjust and/or manage drug regimens for patients, pursuant to a patient-specific written orders or protocols made by the patients' physicians or collaborative practice agreements. Evaluate patients to determine if medical regimen is appropriate and/or detect any symptoms that could be attributed to adverse effects from current therapy. Analyze patients' needs and collaborate with ambulatory care and clinical community to educate patients on all pharmacy services.
Minimum Qualifications
Requirements: Doctor of Pharmacy (Pharm.D.) degree or equivalent, a valid New York State license to practice pharmacy, New York State certification to practice Collaborative Drug Therapy Management (CDTM), and completion of a pharmacy residency training program in a clinical/hospital setting. Salary offered: $154,097.
