Responsibilities
  • Comply with all state-mandated Occupational Therapy Practice Act, Occupational Therapy Code of Ethics and other legal mandates.

  • Evaluate client for therapy needs and provide written orders/treatment plan for services.

  • Ensure plan of care is consistent with client assessment and needs.

  • Instruct other personnel and/or family members and caregivers on intervention goals.

  • Ensure proper visit documentation completion.

  • Report client's response to treatment or changes in condition to DON, ADON and/or client's primary provider.

  • Participate in discharge planning..

  • Provide in-service education for agency personnel, as needed.

  • Ensure personal credentials are active and up to date at office.

  • Attend all staff meetings and complete all required annual education as assigned.

Requirements
  • Current unrestricted Occupational Therapist license in state for which care is being provided

  • One (1) year experience as occupational therapist, preferably in an in home setting

  • CPR Certification, required

  • Two-step negative TB skin test or chest x-ray, required.

  • Level 1 Fingerprint Clearance Card, required

  • Valid Driver's License and auto insurance, required

  • Understands regulatory compliance with state and federal regulations

  • Adheres to HIPAA and maintains client confidentiality

  • Transportation to meet the needs of position and scope of job responsibilities, required
