JOB TITLE:                                     Hospice Aide - Certified Nursing Assistant

RESPONSIBLE TO:                      Director of Clinical Services


POSITION SUMMARY:           

As a member of the interdisciplinary team, the hospice aide provides personal care services to the hospice patient and performs related tasks in the patient/caregiver home in accordance with the plan of care as assigned by the primary nurse. Provides temporary respite and relief from total care responsibilities for the primary caregiver.


ESSENTIAL JOB FUNCTIONS:

  • Provide personal care to the patient in a compassionate and understanding manner consistent with the hospice philosophy while maintaining the dignity of the patient.
  • Perform duties under the written and/or verbal direction of the RN according to the patient’s plan of care:
    • Measure and record vital signs (temp, pulse, respirations, oxygen saturation).
    • Turn oxygen off and on.
    • Provide bed bath, shower, or tub bath as appropriate.
    • Provide care of skin, hair, mouth, and feet (excluding nail care); nail care for hands as directed by the Director of Clinical Services.
    • Ensure good body alignment, positioning, and range of motion exercises for non-ambulatory patients.
    • Assist with patient mobility (crutches, walker, wheelchair, Hoyer, sit to stand).
    • Assist with catheter care, application, and removal, as well as use of bedpan, urinal, commode, and toileting.
    • Assist with ostomy care of an established ostomy.
    • Assist with feeding and dressing patients.
    • Report significant changes in the patient’s condition to the RN immediately.
    • Change simple dressings.
    • Perform housekeeping duties related to patient care.
    • Perform simple procedures as an extension of therapeutic services as delegated by the RN and in accordance with the plan of care.
    • Order and maintain supplies as assigned.
    • Document patient care in electronic medical records, ensuring accuracy, completeness, and compliance with regulations, certification standards, and legal and ethical standards.
    • Identify and address comfort care needs within the scope of the Hospice Aide role.
    • Communicate with and support the patient and caregiver, providing education on best practices for assisting the patient.
    • Support the patient’s and caregiver's unique spiritual and cultural beliefs.
    • Consult and collaborate with the interdisciplinary team and others involved in the patient’s care, including attending Interdisciplinary Team Meetings.
    • Schedule and complete patient visits assigned by the Director of Clinical Services or RN Case Manager.

QUALIFICATIONS AND EDUCATION REQUIREMENTS:

  • Graduate of a state-certified Nurse's Aide training program with active Board of Nursing certification on the Oregon Nursing Assistant Registry.
  • Previous experience in hospice, home health, or long-term care preferred.
  • Ability to work independently with strong written and verbal communication skills.
  • Computer proficiency and working knowledge of Microsoft programs required.
  • Valid Driver’s License and daily access to a reliable and insured vehicle.

  • Category I: Tasks that involve potential exposure to blood, body fluids, or tissues.
