About The Role:

With a mission for quality care towards residents and their families, the RAI Assessor is accountable for performing RAI related responsibilities in accordance with established policies and in compliance with the standards of the Canadian Institute of Health Information (CIHI) and Alberta Health Services (AHS).

What You Will Do:

ESSENTIAL DUTIES AND RESPONSIBILITIES1.Resident Centered Care: Promotes service excellence through active participation with the Interdisciplinary Team to achieve and maintain quality resident centered care within the scope of a RAI Assessor.Conducts RAI-MDS assessments in alignment with Bethany Care policies/procedures and Canadian Institute for Health Information (CIHI) standards for practice.Practices in the RAI Assessor position within the professional role of a Licensed Practical Nurse, adhering to the CLPNA standards (or regulations).Accesses appropriate information and resources that enhance resident care for the achievement of desired resident outcomes (i.e. RAI-MDS assessment, Outcome scores and Clinical Assessment Protocols).Acts as an advocate to protect and promote residents rights to autonomy, respect, privacy, dignity and access to information.Develops and maintains a current RAI- MDS schedule.Communicates with residents, family members and all Interdisciplinary Team members to gather information for the RAI-MDS assessment.Compiles current knowledge of the status for assigned residents (e.g. 24-hour reports, health care documentation, care plans, assessment tools, behaviour maps, consultation reports, test results, team members) toward accuracy of RAI-MDS assessmentsIs a member of the Interdisciplinary Team that assists with collaborative reviews and revisions of care plans based on the residents' assessed Unmet Health Care Needs, related Health Care Goals and Interventions.2.Documentation: Accurate, comprehensive and timely documentation of resident RAI-MDS assessments, in alignment with Bethany policies and procedures.Documents in healthcare records, including contributing to resident centered care plan decisions.Communicates resident changes at Interdisciplinary Team meetings, following completion of the RAI- MDS assessment to ensure data consistency and quality.Completes all RAI-MDS assessments for assigned residents (admission, quarterly, annual and with identified significant change in status), in collaboration with Interdisciplinary Team members, residents and family members.Completes and locks each RAI-MDS assessment per CIHI guidelines.3.Safety: Contributes to a safe environment for residents, the team and self by understanding and adhering to safety guidelines and practices at all times.4.Team Work: Cares for and works cooperatively with others as an effective team member.Promotes constructive caring relationships with the Interdisciplinary Team, by fostering an understanding of the unique contributions of all team members.Collaborates with the Interdisciplinary Team to provide resident centered services.Promotes and maintains a partnership with residents' families.Assists in the review of programs and actively participates in the implementation and evaluation of new programs and services.Contributes to the establishment of short and long term goals for the resident.Works with Health Care Aides to promote documentation accuracy.Liaises with Care Service Managers to share information that facilitates high level clinical decision making. 5.Leadership: Enables a high performance culture through leading, developing and coaching employees.Provides coaching and support to all unit staff in relation to RAI related initiatives.Identifies staff POC/PCC education needs and communicates needs to Care Service Manager.What You Bring:QUALIFICATIONS1.EducationCompletion of Licensed Practical Nurse program.Completion of all RAI training modulesScore of 90% or higher on yearly RAI competency exam2.ExperienceAt least 5 years' of recent work experience with seniors and/or dependent adults in long term care3.Professional RegistrationCurrent Licensed Practical Nurse in good standing with CLPNA4.Additional SkillsAble to work effectively in a physically and mentally demanding environmentMust demonstrate an acceptable level of reading, writing and comprehension of English language skillsStrong observation, assessment, communication, interpersonal, and critical thinking skillsHighly proficient with Point Click CareCurrent CPR
