Manager- CHW

Manager – Community Health Worker (CHW)

As a Manager – Community Health Worker (CHW), you will become part of a patient-centered healthcare environment dedicated to improving access to healthcare services and connecting underserved populations with essential community resources. This role provides the opportunity to lead and develop a team of outreach representatives while supporting patient engagement, preventive care, and positive health outcomes. The Manager – CHW is responsible for overseeing outreach activities, developing effective patient engagement campaigns, monitoring team and program performance, and collaborating closely with clinical staff and other healthcare stakeholders. Through strong leadership, data-driven decision-making, compassionate communication, and effective coordination of care, this position helps ensure that patients receive timely services and the community's healthcare needs are effectively addressed.

A typical day for a Manager – Community Health Worker may include:

  • Managing, leading, and supporting a team of outreach representatives to achieve established outreach, engagement, and campaign goals
  • Developing and implementing outreach campaigns designed to improve patient engagement, healthcare access, and compliance with recommended preventive care measures
  • Monitoring outreach activities and team performance to identify opportunities for improvement and ensure program objectives are met
  • Analyzing outreach and patient engagement data to evaluate campaign effectiveness and make data-driven recommendations for adjustments
  • Collaborating with clinical staff and other healthcare professionals to help ensure patients receive timely and appropriate care
  • Overseeing follow-up efforts with patients to encourage and confirm scheduling and completion of annual wellness visits
  • Identifying patients who may qualify for medical certification and coordinating with clinical staff to support the certification process
  • Monitoring patient outcomes and evaluating the effectiveness of outreach initiatives
  • Preparing and presenting outreach, campaign, and patient outcome reports to senior leadership
  • Maintaining accurate and timely documentation of outreach activities, patient contacts, follow-up efforts, and outcomes
  • Ensuring all outreach activities are conducted in accordance with applicable healthcare regulations, organizational policies, and patient privacy requirements
  • Providing coaching, guidance, motivation, and performance support to outreach representatives
  • Communicating effectively with patients, outreach staff, clinical teams, leadership, and community partners
  • Promoting culturally appropriate, respectful, and patient-centered outreach practices when working with underserved populations
  • Supporting continuous quality improvement initiatives related to patient engagement, preventive care, healthcare access, and community resource coordination
  • Demonstrating strong organization and time management while balancing team supervision, outreach goals, reporting requirements, and patient needs
  • Performing other assigned responsibilities in accordance with departmental and organizational needs

To be considered for this Manager – Community Health Worker opportunity, you should possess a Bachelor's degree in Healthcare Administration, Public Health, or a related field, along with 3–5 years of experience in outreach, patient engagement, community health, or a related healthcare setting. Candidates should demonstrate strong leadership and management abilities, excellent communication and interpersonal skills, and the ability to analyze data and make data-driven decisions. Familiarity with healthcare regulations and patient privacy laws, knowledge of medical terminology and healthcare procedures, and experience collaborating with clinical staff are important for success in this role. Proficiency in Microsoft Office Suite and electronic medical record systems is also required.
