Job description

The MDS Coordinator coordinates interdisciplinary resident assessments and care planning for nursing facility residents while overseeing quality assurance processes. This role ensures timely and accurate completion of Minimum Data Set (MDS) assessments, care plans, and related documentation in accordance with regulatory requirements and organizational standards.

Essential Duties and Responsibilities

  • Coordinate interdisciplinary team assessments and resident care planning processes.
  • Complete, review, and audit MDS assessments for accuracy and timeliness.
  • Identify and calculate Resident Assessment Protocol (RAP) triggers.
  • Interview residents and resident representatives and document findings.
  • Conduct weekly healthcare planning meetings.
  • Monitor resident status changes and ensure appropriate assessment updates are completed.
  • Review and update resident care plans in collaboration with residents, families, and the interdisciplinary team.
  • Participate in admission and discharge planning.
  • Analyze quality assurance data and report findings to leadership and the Quality Assessment and Assurance Committee.
  • Collaborate with department leaders to develop and implement quality improvement initiatives.
  • Provide staff education and training related to assessment and care planning processes.

Qualifications

Required

  • Active Registered Nurse (RN) license.
  • Graduate of an accredited school of nursing.
  • Minimum of one year of experience in a geriatric or long-term care setting.
  • Knowledge of MDS regulations, care planning, and quality assurance processes.
  • Strong organizational, communication, and analytical skills.
  • MDS Coordinator experience.

Preferred

  • Bachelor's degree in Nursing (BSN).
