RN/LPN | Skilled Nursing

Holland Home of Grand Rapids, a 5-Star Skilled Nursing Facility! Holland Home of Grand Rapids is one of Michigan's largest non-profit providers of senior living services. Offering the full continuum of care on 2 beautiful campuses as well as community services through Atrio Home Care, Faith Hospice, Care Resources, Reliance Community Care Partners, and Tandem365!We have about 1,500 team members and serve over 5,000 seniors.

Responsibilities:

  • Delivers compassionate resident care in alignment with the mission of Holland Home
  • Conducts assessments, administers medications, maintains the medication cart, and follows through with physician orders
  • Provides guidance and supervision to CNAs and other assigned staff
  • Assists physicians with evaluations and procedures as directed
  • Ensures timely completion of all necessary documentation
  • Establishes trusting relationships with residents and their families, offering emotional support and addressing any concerns that may arise

  • Schedule:Wk 1: Sun (12), Fri (12) | Wk 2: Sat (12)
  • Shift: 6:30pm - 7am | 36 hrsper pay period
  • Schedule:Wk 1: Fri, Sat (12) | Wk 2: Fri (12), Sat
  • Shift: 6:30pm - 10pm/7am | 40 hrs per pay period

Qualifications and Requirements:

  • A current and valid MI RN/LPN license is required
  • Must be CPR certified
  • Prior experience with geriatrics preferred but not required as there is a lot of support on the units
  • Basic computer skills to chart in the EMR
