Job description

  • Primary function to triage phone calls
  • Assists patients with activities of daily living, including bathing, dressing, grooming, toileting, positioning, linen changes, meal preparation, feeding assistance, intake/output monitoring, rounding, and responding to call lights to ensure safety and comfort.
  • Supports patient activity programs and assists with admission, transfer, and discharge processes, including admission histories with RN validation.
  • Performs delegated clinical tasks per nursing direction, including EKGs, IV removal, vital sign equipment checks, and other nursing functions as assigned.
  • Monitors patient conditions, reports changes, accidents, and incidents, and communicates patient needs to the assigned nurse.
  • Documents care, patient information, and charges accurately in electronic health records and logs.
  • Conducts bedside registration by interviewing patients or responsible parties and entering demographic, insurance, and legal documentation (e.g., living wills, DPOA).
  • Provides clerical and administrative support, including answering and triaging calls, routing messages and lab results, maintaining charts, census and medication reports, and managing office supplies.
  • Greets and directs visitors and maintains effective communication with patients, families, staff, physicians, and ancillary departments.
  • Ensures availability of patient care equipment and reports shortages to nursing leadership.
  • Upholds patient rights and supports the mission, values, policies, and professional standards of CHCS.
  • On-going compliance with CHCS educational requirements.
  • All other duties as required.
