Job description

Licensed Practical Nurse For Home Care

Shift: Day shift - 8:00am - 4:30pm, Monday - Friday w/rotating weekends

Blue Team – Brattleboro/Ludlow, VT Service areas will also include: Ludlow, Cavendish, VT areas.

Position Objective:

Home Care Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.

Performance Expectations:

Under the supervision of the RN, provides ongoing evaluation of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals. Under RN supervision may continue education of medication, wound care and disease management. Shares all findings with the Case Manager and the RN as fitting.

Performs procedures based on the State and Agency's policies and procedures after successful completion of Agency's competency testing.

Provides nursing care in a way that considers patient's privacy, comfort and safety. All care is conducted following the VHN policies and procedure after successful VNH competency testing is achieved.

Participates in the coordination of the patient's care plan through interdisciplinary communication and case conferences, as appropriate.

Completes and submits accurate and relevant clinical notes and other required documentation in a timely manner in accordance with expected agency standards, state and federal regulatory compliance i.e., clinical notes; day sheets; status change; payer source documentation, including monitoring of authorizations; discharge paperwork.

Completes all clinical documentation on the laptop and in the home when appropriate and in a timely fashion. Transmits all information twice a day.

1) Weekends – All Home Health nurses will be on a rotating weekend schedule to ensure we have adequate staffing coverage.

2) Holidays – All Home Health nurses will be on a rotating holiday schedule to ensure we have adequate staffing coverage.

VNH is committed to ensuring the safety of all our travelers without causing any negative impact. The VNH will take full responsibility for covering the cost of all mandated COVID tests required by agency protocols or state regulation.
