Job Description

A Home-Based Case Worker provides and connects families and children with the necessary tools and resources to sustain their everyday needs. As a provider, it is essential to assess the needs of each client in order to obtain necessary goals, and to complete tasks. Home-Based Case Work also includes helping people improve on making/obtaining goals, child care, coping skills, life skills, hygiene, employment, housing, communication, enhancing resilience, healthy relationships, safety, conflict management, problem solving, advocacy, budgeting, parent education, and knowledge of community resources. Mentorship and advocacy are very essential for this position to ensure the client is being properly supported and assisted. A Visit Facilitator is to protect the integrity of the visit and to provide a positive atmosphere where parents and children may interact in a safe structured environment in the home, if permitted, or in a safe public area in the community. This visit should be supervised by the facilitator to make sure the parent and child are bonding, reconnecting/reestablish, and strengthening the parent-child relationship dynamic in an appropriate manner. Conversations during visits or in transport concerning addresses, foster placement, or the case are strictly prohibited..

Essential Duties and Responsibilities 

● Create a consistent schedule for parent and child to meet weekly.

● Transport child(ren) to visiting parent in the community or in the home if permitted.

● Understand the levels of Supervised Visitation.

● Complete Supervision intake Assessment with visiting parent.

● Ensure the Client Handbook is read, understood, and signed by the visiting parent.

● Complete Protective Factors Survey every 90 days.

● Always pick up the child first, and drop off the parent first.

● Ensure there is no discussion about the case, address of homes or school, or foster placement.

● Document pertinent info on each visit within 24 hours of the visit thoroughly and in detail.

● Create a safety plan with visiting parent.

● Monitor, document, report, and address any safety concerns in Case Management Pro.

● Complete a monthly report by the 10th of every month.

● Maintain confidentiality within CSS policies and procedures.

• Assess the needs of the child or family in order to make short and long term goals to obtain
• Provide and connect the child and or family with necessary community resources
• Provide mentorship and advocacy to ensure the client is being supported accordingly
• Document each session in Case Management Pro within 48 hours of meeting.
• Document visits and monthly reports in detail via Case Management Pro.
• Create a safety and treatment plan in Case Management Pro.
• Ensure the Client Handbook is read, understood, and signed by the client.
• Assist with improving caretaking and coping skills, mental/physical/emotional support, and tactics in order to improve family interaction and well-being.
• Monitor, document, report, and address any safety concerns in Case Management Pro.
• Document evidence based models used in each session in Case Management Pro.
• Create safety and crisis response plans for record keeping.

Educational Requirements/Experience 

Paraprofessional Level Direct Worker

● High school diploma or GED

Bachelor’s or Master’s degree in a directly related human services field. The individual must possess one of the following:

● Bachelor’s or Master’s Degree from an accredited university

● High School Diploma or equivalent, or Associate’s Degree

● This employee must also have at least 4 years full time employment experience providing direct service to children and families

● Experience must include service to families that need assistance in the protection and care of their children

● Worker must be trained and competent to complete the services as required by federal and State of Indiana Law

● Worker must be credentialed according to the requirements of the evidence-based model(s) used

Must have training in the following to complete successful documentation:

Motivational Interviewing (HBCW)

General Requirements 

• Must have ability to work a flexible schedule (days/nights/weekends)
• Must maintain CPR, TB and First Aid certification, and must stay current on all other training as defined by Indiana state guidelines, industry accreditation standards and company guidelines.
• Must complete a successful background check upon hire (prior to providing care to youth) and at a minimum of annually.
• Must have knowledge of family of origin/ intergenerational issues
• Must have knowledge of child abuse/neglect
• Must have knowledge of child and adult development
• Must have ability to work as a team player
• Must have ability to properly assess crisis situations and provide redirection
• Must possess a valid driver’s license and the ability to use a personal car to transport self, and others. Must also comply with the state policy concerning minimum car insurance coverage
• Must be trained and competent to complete the service as required by federal and State of Indiana law.
• Direct Workers must be credentialed according to the requirements of the evidence-base model(s) used
• Must carry appropriate caseloads and should not exceed 12.
• Belief in the family preservation philosophy
• Skillful in the use of evidence-based strategies
• Must be atlease 21

Certifications/Medical Records

• TB test (annually)
• First Aid certification
• CPR certification

Communication/Reporting 

• Must be in contact with the client within 48 hours of accepting the referral.
• Must explain, maintain and uphold CSS Client Handbook policies and procedures.
• Must maintain confidential information in accordance with HIPPA & Cornerstone Guidelines.
• Must maintain proper documentation with conversations, emails, and texts.
• Must ensure that all communication is professional and consistently reflects dignity and respect.
• Must immediately alert the supervisor of any critical or potentially critical issue or crisis.
• Must prepare, maintain, and provide any statistical reports, program reports, or other information as requested by DCS relating to the services provided.
• Must complete monthly reports by the 10th of the month following service.
• Must submit reports into Case management Pro 24 hours after each session which should be put into KidTraks within 48 hours.
   
  Self Management Skills 
• Must maintain consistency and punctuality with scheduled appointments and meeting times.
• Must document accurate records of attendance.
• Must organize and prioritize schedules and keep track of tasks.
• Must demonstrate cooperative attitude and tolerance for stress when confronted with critical situations.
• Must maintain confidentiality, integrity, and accountability within the scope of responsibilities

Documentation 

● Outcomes of the visit (if held, or not held/canceled)

● Name of Supervision Provider Specialist

● Date, location, and level of supervision visit

● Participants in attendance at the visit

● Time of arrival and departure of all parties for the visit

● Greetings and departure interaction between parent and child

● Positive interactions between parent and child

● Planned activities by the parent for the visit

● Interventions required, if any, and parent’s response to direction provided with regard to interventions

● Ability and willingness of parent to meet the child’s needs as requested by child or facilitator

● Tasks he parent to be completed prior to or at the next visit

● Pertinent information, issues, or concerns regarding the child’s placement

● Note how the parent demonstrated each of the following:

Business and Technical Skills 

● Ability to apply general understanding and knowledge to carry out instructions in written, verbal, or schedule form.

● Ability to handle and carry out multiple tasks.

● Basic computer skills, with the ability to learn and effectively utilize identified computer software applications to accomplish tasks.

● Ability to read, comprehend, interpret and apply information from general procedures, safety rules, instructions, data and reports.

● Ability to use basic English, the grammatical skills to write routine reports, and general correspondence.

Organizational skills, and the ability to accurately document data and maintain records.
