Job description

Position Title:                      Case Manager/Interpreter                     

Status:                              Full-time permanent

Location:                          24 Selkirk Street, Ottawa, ON

Reports to:                       Executive Director        

Akausivik Inuit Family Health Team (AIFHT) is a not-for-profit medical clinic providing culturally safe, community-based primary medical care to Inuit individuals and their families. We work closely with federal, provincial, and local health partners to deliver holistic, respectful, and responsive care grounded in Inuit cultural safety principles.

We are seeking a Case Manager/Interpreter to join our team. Working conjointly with the interdisciplinary team to offer one-on-one care, the Case Manager/Interpreter provides medical interpretation (Inuktitut and English) and acts as liaison to individuals and families. Case Managers/Interpreters organize small groups for education and social support, and facilitate community service integration. This includes working closely with patients and families, as well with service providers to help families meet their needs, overcome obstacles and reach their goals.


Key Responsibilities:

  • Build community trust and relationships, and facilitate cross-cultural understanding with service providers.
  • Case Management and care navigation, including facilitating communication (language and culture interpretation).
  • Guide resolution of issues, regarding access and barriers to health.
  • Provide individual and family support, including linking families to services, resources and supports that they need.
  • Facilitate comprehensive, seamless care as patients navigate AIFHT, hospital, and community services.
  • Explore community resources and facilitate referrals for patients.
  • Maintain required patient information records, data entry, documentation, and action items, while demonstrating a high level of confidentiality.
  • Other related duties and activities as may be assigned.

 Qualifications:

  • Post-Secondary Education in Social Work or Human Services, with 2-3 years of community health experience with the Inuit community. A combination of relevant education and experience will be considered.
  • Demonstrated knowledge of Inuit health issues and culture.
  • Knowledge, training and/or experience with health promotion and disease prevention, as well as access to resources within or outside of their community.
  • Ability to prioritize, manage time effectively, and be flexible in an inter-professional team environment.
  • English and Inuktitut communications skills are required, including medical terminology and Inuktitut interpretation thereof.
  • Knowledge of risk assessment tools and treatments plans.
  • Competency in computer-based programs (Microsoft Programs).


Working Conditions:

  • Must be accessible by phone.
  • Valid driver’s license and access to a reliable vehicle.
  • Occasional evening and/or weekend work may be required.
