POSITION SUMMARY:
  • The RN is experienced in planning, organizing, and directing home health services with an emphasis on holistic care.
  • The professional RN builds from resources to plan and direct services to meet the needs of individuals and their family members within their homes and communities.
  • Completes an initial assessment of the patient and family to determine home care needs, including a complete physical assessment and history of current and previous illnesses.
  • Develops a care plan involving the patient and the family to establish goals based on nursing diagnosis, incorporating therapeutic, preventative, and rehabilitative nursing actions.
  • Provides direct patient care as defined in the State Nurse Practice Act.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
  • Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNAs, and MSWs.

QUALIFICATIONS:
  • An associate's or Bachelor’s degree with one year of home health care experience preferred.
  • AZ Nursing licensure in good standing.
  • Current CPR/BLS certification.
  • Fingerprint clearance card.
  • TB test results within the last year.
  • AZ Driver's license and proof of insurance with reliable transportation.
  • Excellent observation, verbal and written communication, and problem-solving skills.
