About The Position
The Case Manager shall perform the services below under the general direction of the Prince George's County Department of Corrections and the direct supervision of the Rebirth Substance Use Treatment (RSUT) Program Clinical Coordinator and Behavioral Health and Wellness Services Section Chief:
Maintain all certifications and educational requirements that are necessary to provide clinical case management in the State of
Complete program enrollment and screening/assessment tools.
Develop person-centered continuity of care plans to outside services when appropriate and facilitate a continuum of care.
Make appropriate referrals based on screening/assessment results.
Follow up with departmental team providers and community-based agencies.
Submit client status reports/progress reports as required.
Maintain appropriate HIPAA compliant electronic case files for all clinical cases.
Participate in regular clinical supervision with Clinical Coordinator.
Respond to inmate requests for face-to-face consultations as
Participate in training on mental health and substance use related issues.
Willingly and conscientiously adheres to the established County Employment Code of Ethics, the ACA codes of ethics and to diligently and responsibly uphold the County's high standard of customer service.
Willingly and cooperatively perform tasks and duties assigned which may not be specifically listed in this contract but are within the occupational category and responsibility level typically associated with the employee's class of work.

MINIMUM QUALIFICATIONS
