Program: Homebase Service Area 6 | 1780 Grand Concourse, Bronx, NY 10457ย 

What You'll Do

As part of New York Cityโ€™s plan to end homelessness, the Department of Homeless Services established Homebase Homeless Prevention Community Resource Centers throughout New York City. Homebase programs are designed to assist families and individuals who are homeless or at imminent risk of becoming homeless to develop a plan for long-term housing stability. Homebase helps clients navigate the complicated eviction process, offers financial and money management counseling, helps clients obtain public benefits, provides short-term financial assistance, and helps with relocation if necessary.

As a Senior Case Manager for one of our Homebase programs, youโ€™ll provide emergency crisis intervention to individuals and families facing an imminent housing crisis and who are at risk of shelter entry or re-entry. Youโ€™ll also provide immediate assistance to urgent and time-sensitive referrals from the NYC Department of Homeless Services in an effort to prevent shelter entry or re-entry. Additional responsibilities include field work, community outreach, and presentations to community-based organizations, as well as providing support to the Case Management Supervisor and the Case Management Team.

Your responsibilities will include:

  • Managing a high-risk and crisis-driven caseload, with constant regard to limited time constraints.

  • Conducting emergency assessments and program intakes, providing appropriate urgent referrals for eligible clients, and providing immediate advocacy and assistance with obtaining public and private benefits to sustain housing stability.

  • Working closely with the Homebase Housing Court Liaison to ensure client assistance during court proceedings.

  • Coordinate with the Case Management Supervisor via case conferences, case consultations, and case record reviews to ensure a high standard of service delivery and program compliance with all regulatory guidelines regarding client care.

  • Assisting with the identification of service delivery gaps and developing service linkages.

  • Supporting and mentoring Case Managers by providing leadership and peer counseling, and co-facilitating trainings with supervisory team members.

  • Conducting Homebase presentations at various community-based organizations.

Youโ€™re a great fit for this role if you have:

  • Bachelor's degree in social work or a related field OR equivalent experience and skills.

  • Knowledgeable of relevant state and county regulations.

  • A minimum of two yearsโ€™ experience in case management, assessment, counseling, and crisis intervention preferred.

  • Strong verbal, written, and negotiation skills, including experience in advocacy and mediation.

  • Strong organizational and documentation skills.

  • Ability to take the initiative, be dependable and resourceful, and pay keen attention to detail.

  • Excellent organizational skills to enable the successful management of multiple tasks promptly in a fast-paced and demanding environment.

  • Knowledge of the family systems approach to practice is preferred.

  • Knowledge of team concepts preferred.

  • Computer literacy, particularly with Microsoft Office applications.

  • Able to provide evening and weekend coverage as needed.

  • Valid US driverโ€™s license is a plus.
