Our main campus is located in Denison, Texas, approximately one hour north of the Dallas/Fort Worth area and just south of the Texas/Oklahoma border. Under the direction of the Director, Case Management, this position is responsible to collaborate with patients, patient family members, physicians and other healthcare professionals to design, implement, and evaluate plans of care specific to each patient's needs, maintaining high quality, cost effective patient care.
Qualifications
Requirements
Education - Per RN licensure requirements. Bachelor's Degree preferred.
Experience At least three (3) years previous clinical and/or case management/utilization management experience preferably in a high volume acute care hospital/healthcare facility.
Licenses - Valid, current RN state license or compact.
Must demonstrate excellent interpersonal, communication and problem solving skills necessary to gather and exchange data, both internally and externally) with key medical professionals, patients and patient family members.
Able to effectively utilize critical thinking and problem resolution skills. Must be able to effectively communicate with key community stakeholders in order to appropriately assist patients
