Your future role at a glance

Location: Evansville, IN

Facility: Ascension St. Vincent

Department/Specialty: Case Management

Schedule: Full Time | Day Shift Mon-Fri | 8am - 4:30pm

  • Empower and lead a dedicated team of case management professionals, fostering a culture of excellence and ensuring every associate has the resources to deliver high-impact patient support.

  • Advocate for clinical integrity by partnering with hospital administration and medical staff to ensure every patient receives the right care, at the right time, for the appropriate duration.

  • Mentor your team as a subject matter expert, providing the technical guidance and clinical insight needed to navigate complex insurance reviews, appeals, and documentation.

  • Optimize daily operations and staffing workflows to ensure seamless unit coverage, balancing the workload so the team can focus on what matters most: successful patient outcomes.

  • Resolve complex care barriers by stepping in as a hands-on resource for the most challenging cases, ensuring no patient's transition plan is left to chance.

What minimum qualifications you'll need

Licensure / Certification / Registration:

  • One or more of the following required:
    • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or jobtransfer date.
    • Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board obtained prior to hire date or job transfer date.
  • Case Manager preferred.
  • Case Manager credentialed from the American Case Management Association (ACMA) preferred.
  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Education:

  • High school diploma/GED with 2 years of experience, OR Associate's degree, OR Technical degree required. Master's degree preferred.

Work Experience:

  • 3 years of experience required.
  • 1 year of leadership or management experience required.
What additional requirements you'll need

Master's SW - LCSW preferred
