Job description

Case Management Assistant
Mather, CA
Schedule: On site | 7:30 AM – 3:30 PM PST | Rotating weekends, holidays required
Note: MUST be legally authorized to work in the United States. 
SUMMARY / DUTIES:

  • Supports case management and Transition of Care (TOC) discharge planning for ambulatory patients by coordinating activities across the care continuum to ensure timely discharges.
  • Partners with Case Managers and Social Workers to assess patient needs, communicate care plans, and coordinate post-discharge services, referrals, and authorizations.
  • Manages high-volume administrative tasks including fax, voicemail, and data entry while maintaining accurate documentation in Epic and other EMR systems.
  • Facilitates patient transfers to post-acute facilities, prepares required documentation, and communicates updates to internal and external stakeholders. Participates in daily huddles, escalates barriers to discharge, and ensures compliance with HIPAA and departmental standards.


REQUIREMENTS:

  • High School Diploma or equivalent experience
  • 1+ years of recent relevant experience in care coordination, case management support, or discharge planning
  • Experience with Epic EHR (required)
  • Experience with utilization review (UR) transmission
  • Experience with high-volume fax and voicemail management
  • Experience with data entry in a healthcare setting
  • Ability to work PST hours regardless of location
  • Ability to work rotating weekends and required holidays
  • Ability to work independently in a fast-paced environment with shifting priorities


PREFERRED REQUIREMENTS:

  • Experience supporting Transition of Care (TOC) or discharge planning workflows
  • Experience coordinating referrals, authorizations, and post-acute placements
  • Experience with Allscripts or similar EMR systems
  • Proficiency in documenting care coordination notes in EMR systems
  • Strong written and verbal communication skills
  • Strong time management and prioritization skills
  • Ability to collaborate with Case Managers and Social Workers
