Job description

LPN / RN - Home Health Care Nurse
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) for a private duty home care case in Mertztown, PA. This position involves providing skilled nursing care and hands-on support for a patient with ongoing respiratory and personal care needs in the home setting.
Responsibilities:
  • Monitor oxygen levels and respiratory status
  • Administer breathing treatments and prescribed medications
  • Provide skilled nursing assessments and ongoing patient monitoring
  • Assist with turning and repositioning to maintain comfort and prevent skin breakdown
  • Monitor and maintain skin integrity
  • Provide incontinence care as needed, including changing briefs and ensuring clean bedding
  • Maintain accurate documentation of care provided and patient status
  • Communicate changes in condition to the appropriate parties
Requirements:
  • Active Pennsylvania LPN or RN license
  • Experience with respiratory care and oxygen monitoring preferred
  • Comfortable providing total care and hands-on assistance in a home setting
  • Reliable, compassionate, and attentive to patient needs
  • Current CPR certification preferred
Schedule:
  • Friday through Sunday
  • 3:00 PM - 11:00 PM
