Prime Time Healthcare Nursing is seeking a travel nurse RN Hospice for a travel nursing job in Hyannis, Massachusetts.

Job Description & Requirements

Hospice RN โ€“ Cape Cod, MA

Position: Hospice RN โ€“ Float
Coverage Area: Cape Cod, Massachusetts
Setting: Home Hospice / Facility-Based Hospice
License: Active Massachusetts RN license required
Experience: 2 years of Hospice RN experience required

Assignment Details
  • Float position covering patients throughout Cape Cod
  • Travelers must be comfortable driving to patientsโ€™ homes across the full service area
  • Coverage includes locations across the bridge and east through the Yarmouth, Harwich, and Orleans areas
  • Reliable transportation is essential
  • Provides hospice care in both patientsโ€™ homes and facility settings
Required Experience
  • Minimum 2 years Hospice RN experience
  • Strong hospice assessment, symptom management, and end-of-life care experience
  • Must be comfortable independently managing patients in the home setting
  • Must pass the VNA pre-hire medication test
Clinical Responsibilities
  • Assess hospice patients and recognize significant changes in condition
  • Provide pain and symptom management
  • Implement and manage individualized plans of care
  • Communicate changes in condition with physicians and other care providers
  • Educate patients, families, caregivers, and facility staff regarding hospice care
  • Complete hospice admissions as needed
  • Obtain and clarify physician orders
  • Provide timely, detailed skilled nursing documentation
  • Participate in interdisciplinary case conferences
  • Supervise Hospice Aides every 14 days per regulatory requirements
