Per Diem/Contingent Physical Therapist – Skilled Nursing Facility

Location: Dearborn

Schedule: Per diem

Join our team at Allegria Village, where we are committed to providing exceptional, compassionate care to our residents. We are currently seeking a dedicated and motivated Per Diem/Contingent Physical Therapist to join our Skilled Nursing Facility team.

Key Responsibilities
  • Provide direct patient care and perform thorough functional evaluations to develop individualized treatment plans
  • Design, implement, and adjust plans of care to achieve optimal patient outcomes, incorporating family participation when appropriate
  • Ensure all therapy practices align with established standards of care and facility policies
  • Supervise and guide support staff to help patients meet their functional goals
  • Collaborate effectively with theinterdisciplinary care team to promote positive outcomes
  • Maintain compliance with facility and departmental policies, including quality improvement, safety, and infection control protocols
  • Complete and maintain accurate documentation, including evaluations, goals, plans, and physician prescriptions
  • Participate in continuing education, workshops, and professionaldevelopment activities to enhance skills and stay current with best practices
  • Foster positive relationships with patients, families, and colleagues to ensure a supportive care environment
  • Perform other related duties as assigned
Education & Certification Requirements
  • Graduate of an accredited Bachelor’s, Master’s, or Doctorate program in Physical Therapy
  • Current license in good standing with the State Board of Physical Therapy Examiners
Qualifications
  • Knowledge and skills specific to the Skilled Nursing Facility (SNF) population
  • Previous SNF experience preferred
  • Understanding of all applicable state, federal, and professional standards
  • Strong problem-solving, organizational, and communication skills (both verbal and written)
  • Demonstratedcompassion, empathy, and professionalism when working with residents and families
  • Excellent customer service orientation and ability to work collaboratively in a team environment
  • Pleasant, positive demeanor and a commitment to delivering quality care
