Job description

Case Manager needed for part-time, flexible position in Bucks County.

We are seeking a compassionate and organized Case Manager to provide supportive case management services to youth and their families throughout Bucks County. Responsibilities include conducting assessments, developing individualized service plans, coordinating community resources, monitoring client progress, maintaining documentation, and collaborating with families, schools, referral sources, and community partners to support positive outcomes.

This is a flexible 30-hour-per-week position, with work hours adjusted as needed to meet the needs of clients and families.
