Job description

Position Summary 

The Per Diem Registered Nurse (RN) at NOHN supports the Nursing Services Department in a variety of areas in an ambulatory care setting. The per diem nurse may provide services including clinical triage, direct patient care, medication management, transitional care management, vaccinations, or care coordination. The RN works closely with providers, patients, pharmacies, and community agencies to support high-quality patient care, improve outcomes, and ensure continuity of care. This role may also provide direct patient visits for specialized services including wound care and Office-Based Addiction Treatment (OBAT) support, based on clinic needs and competency.  

This position offers a flexible opportunity for an RN to support patient care across triage, care coordination, medication management, transitional care, and direct clinical services in a collaborative outpatient setting. 

Essential Duties and Responsibilities 

Clinical Triage 

  • Perform telephone and in-person patient triage using established protocols and clinical judgment. 

  • Monitor EMR messages and respond to urgent patient needs. 

  • Collaborate with providers and care teams to ensure timely patient evaluation and treatment. 

  • Coordinate appropriate scheduling and follow-up based on patient needs. 

  • Serve as a clinical resource for staff and escalate urgent concerns to providers. 

Medication Management 

  • Process and monitor medication refill requests. 

  • Verify patient information, medication history, and refill eligibility. 

  • Communicate with pharmacies and providers to resolve medication-related issues. 

  • Maintain accurate medication records in the EMR. 

  • Educate patients on medication use, adherence, and follow-up care. 

Transitional Care Management (TCM) 

  • Conduct post-discharge outreach and assessments following hospitalization or emergency department visits. 

  • Review discharge instructions, medications, and care plans with patients. 

  • Coordinate follow-up appointments, referrals, and community resources. 

  • Monitor patient progress and communicate concerns to providers. 

  • Document transitional care activities in accordance with regulatory requirements. 

Direct Patient Care 

  • Provide nursing visits and clinical services within scope of practice. 

  • Perform wound assessments, wound care treatments, and patient education as appropriate. 

  • Support Office-Based Addiction Treatment (OBAT) programs, including patient education, monitoring, care coordination, and collaboration with prescribing providers. 

  • Administer medications and complete required documentation. 

  • Assist with other clinical procedures and patient care activities as assigned. 

Documentation and Care Coordination 

  • Maintain accurate, timely, and complete documentation in the electronic medical record (EMR). 

  • Coordinate care with providers, specialists, pharmacies, hospitals, and community agencies. 

  • Ensure compliance with HIPAA, nursing standards, organizational policies, and regulatory requirements. 

  • Participate in team meetings, quality improvement activities, and other duties as assigned. 

Qualifications: 

  • Education: Graduate of an accredited school of nursing required. BSN preferred.  

  • Experience: Current, active WA State Registered Nurse license required. Certified Basic Life Support required.  

  • Skills: Proficiency in Electronic Health Record systems, Ability to utilize computers for data entry and information retrieval, excellent verbal and written communication skills, Demonstrate a high level of skill at building relationships and customer service. Demonstrate influential skills in managing difficult clients and patients. Demonstrate a high degree of knowledge and competency in the practice of medicine and associate charting requirements. Requisite skills and ability to perform certain medical tasks as assigned. Demonstrate a high level of problem-solving skills to better serve patients and staff. Strong organizational skills, ability to work independently and as part of a team, cultural sensitivity. Ability to implement and evaluate operational and administrative processes. Uphold the mission and values of the organization. Create and maintain a positive culture.   

  • Language: Written and spoken English required.  

Physical Demands: 

  • Will require prolonged standing and sitting in front of a computer. Normal accessibility and mobility required.  

Work Environment: 

  •  Fast-paced community health center setting, potential exposure to infectious diseases, interaction with diverse patient population.
