LVN- Night Shift

Job Summary:

Bronte Health & Rehab Center is seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our team for a full-time night shift position. As an LVN at our center, you will play a vital role in providing high-quality care to our patients and residents. Our efficient and dedicated staff is dedicated to making each patient feel like family, and we strive to create a warm and welcoming environment for everyone who walks through our doors.

Responsibilities:

  • Provide direct patient care, including administering medications, wound care, and treatments, under the supervision of a Registered Nurse (RN)

  • Collaborate with the RN to develop and implement individualized care plans

  • Monitor and report patient conditions, as well as changes in their status, to the RN and other healthcare team members

  • Participate in interdisciplinary team meetings to coordinate patient care

  • Ensure all patient care is delivered in accordance with the center's policies and procedures

  • Maintain accurate and thorough records of patient care and treatment

  • Promote a welcoming and inclusive environment for patients, families, and staff
