Registered Nurse Case Manager - Home Care

BrightStar Care is expanding in the South Minneapolis area and is seeking a Registered Nurse (RN) to serve as a Home Care Case Manager four days per week. This salaried role works closely with a team of RNs and LPNs to complete assessments, reassessments, and select skilled nursing tasks for private-pay and veteran clients. As a non-Medicare agency, BrightStar Care does not require OASIS documentation. Our nursing team is collaborative, supportive, and focused on helping clients remain safe and comfortable in their homes. This position offers PTO accrual starting on day one and paid mileage between client visits and the office. Most clients will be located within the Minneapolis metro area (Bloomington, Edina, Eden Prairie, and surrounding communities), allowing you to work close to home.
