If you're an RN who enjoys providing exceptional patient care but is ready for more variety, professional growth, and leadership opportunities, this may be the position you've been looking for. BrightStar Care of Lake County is seeking an experienced Registered Nurse to join our growing team in a unique hybrid role that combines approximately 50% field nursing with 50% office-based clinical leadership and support. Unlike traditional home health positions focused solely on patient visits, this role allows you to build lasting relationships with clients while also contributing to admissions, caregiver development, quality assurance, and clinical operations. This is an excellent opportunity for an RN who enjoys both direct patient care and being part of a collaborative leadership team. What You'll Do Skilled Nursing & Clinical Care You'll provide high-quality nursing care while serving as a clinical resource for our clients and caregivers. Responsibilities include: * Perform nursing assessments, admissions, reassessments, and supervisory visits * Provide skilled nursing care in clients' homes * Perform wound care and monitor healing progress * Manage and support infusion therapy patients * Identify changes in patient condition and communicate appropriately with physicians, families, and the care team * Educate clients and families regarding medications, treatments, and disease management * Develop, implement, and update individualized care plans * Maintain flexibility to complete client assessments, admissions, and supervisory visits outside of regular business hours when necessary to meet client needs. This may occasionally include evening or weekend visits. Clinical Operations & Office Support As an important member of our clinical leadership team, you will also: * Review clinical documentation for accuracy and regulatory compliance * Process and track physician orders * Assist with new client admissions and referral coordination * Maintain accurate client records and clinical documentation * Conduct caregiver skills assessments and competency evaluations * Assist with caregiver orientation and clinical training * Collaborate with scheduling, recruiting, and leadership to ensure exceptional client care * Communicate proactively with the Director of Nursing regarding client care needs and operational priorities What Makes This Position Different?
