Responsibilities * Comply with all state-mandated Physical Therapy Practice Act and other legal mandates. * Evaluate client for therapy needs and provide written orders/treatment plan for services. * Ensure plan of care is consistent with client assessment and needs. * Instruct other personnel and/or family members and caregivers on intervention goals. * Ensure proper visit documentation completion. * Report client’s response to treatment or changes in condition to DON, ADON and/or client’s primary provider. * Participate in discharge planning.. * Provide in-service education for agency personnel, as needed.
