Job description

Responsibilities
  • Provide Population Health Management Services to patients of the Medical Group of ChristianaCare
  • Complete comprehensive medication reviews for eligible patients
  • Ensure performance targets for medication-related measures are met or exceeded
  • Collaborate with providers and patients
  • Design and implement clinical programs
  • Provide comprehensive medication management services and annual wellness visits to eligible patients
  • Utilize risk stratification tools to identify targeted highโ€‘risk patient population
  • Review for contraindications/precautions
  • Document drug use and compliance patterns, adverse drug effects
  • Coordinate activities with health care team to perform enhanced medication reconciliation
  • Participate in research design, data analysis, and publication of study results
  • Contribute to the pharmacy and medical literature
  • Provide an interprofessional experience in training and mentoring pharmacy students and residents
Requirements
  • PharmD from an ACPE accredited School of Pharmacy Required
  • ASHP-accredited PGY-1 Pharmacy Practice Residency required or equivalent experience (3 years of clinical pharmacy experience)
  • ASHP-accredited PGY-2 Ambulatory Care Residency preferred
  • Experience in ambulatory care and/or population health preferred
  • BPS board certification preferred
  • Current licensure (or eligible to reciprocate) in the State of Delaware
  • May require licensure in additional states in order to provide virtual care to patients.
