Job description

This role will be responsible for providing Home Health care to patients in the local area in the comfort of their own home!

Responsibilities

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Provide compassionate care by conducting health assessments and crafting personalized treatment plans.
  • Administer and monitor medications while educating clients and staff on proper care.
  • Utilize your experience with clinical assessment, psychosocial rehabilitation, and bio-psychosocial therapies to make a difference.
  • Use your knowledge of electronic health records and data analytics to ensure optimal care and wellness outcomes for our clients.

Requirements:

  • 1+ year of professional experience as an RN (Home Health experience preferred)
  • Active RN License in the State of PA Associates Degree (or higher) in Nursing
  • BLS and CPR Certification (if you don't have this - we will help you get it!)
  • Valid Driver's License, Reliable Mode of Transportation, and Auto Insurance

Salary: $80,000 annually. This position is full-time.
