Responsibilities:

  • Assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals in a community setting.
  • Collaborate with healthcare team members to develop and evaluate care plans and services.

  • Manage medication administration, monitoring, and disposal in compliance with policies.
  • Monitor patient conditions and report changes to physicians promptly.
