Field Registered Nurse (RN) โ€“ Home Health

PRN

Overview:
We are seeking a compassionate and skilled Field Registered Nurse (RN) to provide high-quality home health services to Medicare patients in their homes. This weekend-based role offers flexible scheduling and is available as PRN, Part-Time, or Full-Time, allowing you to choose an option that best fits your lifestyle and availability.

As a Weekend Field RN, you will deliver patient-centered care, perform skilled nursing visits, and collaborate closely with our clinical team to ensure safe, effective, and timely care.


Key Responsibilities:
  • Provide skilled nursing care to patients in their homes in accordance with the plan of care and Medicare home health guidelines.

  • Perform weekend patient visits, including Start of Care (SOC), Resumption of Care (ROC), routine visits, and follow-up assessments.

  • Complete accurate and timely clinical documentation.

  • Monitor patient conditions, report changes to the physician and care team, and ensure continuity of care.

  • Educate patients and caregivers on disease management, medication use, safety, and home care plans.

  • Communicate regularly with clinical leadership, therapists, CNAs, and other team members.

  • Maintain compliance with agency policies, clinical standards, and regulatory requirements.

Qualifications

  • Active RN license in the state of Florida.

  • Minimum of one year of nursing experience (home health experience).

  • Strong clinical assessment and critical-thinking skills.

  • Ability to work independently in the field and manage a weekend caseload.

  • Reliable transportation, valid driver's license, and current auto insurance.

  • Excellent communication and documentation skills.
