Job Summary:

We are seeking a highly motivated and experienced Registered Nurse (RN) to provide exceptional patient care to our patients in the safety and comfort of their homes. As an RN with Home&Care, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities:

  • Provide direct patient care, including assessment, planning, implementation, and evaluation of care for our residents

  • Develop and implement individualized care plans, in collaboration with the interdisciplinary team

  • Administer medications, treatments, and therapies as prescribed by physicians

  • Monitor and document patient progress, identifying and reporting changes in condition
