Position Details:
  • Main Office location: Worcester, MA
  • Service Area: Worcester, Sturbridge & Surrounding Areas
  • Job Type: Per Diem
  • Schedule: Flexible
  • Per Visit Rates Apply

Responsibilities:
  • Conduct admission assessments and develop personalized treatment plans.
  • Provide routine and urgent care visits, ensuring flexibility to meet patient needs.
  • Manage medication orders and educate patients on proper usage.
  • Handle wound care, testing, and equipment/supply needs.
  • Document patient progress and collaborate with the care team for optimal results.
  • Assist in discharge planning and coordinate with community resources.
  • Participate in team meetings and briefings to ensure cohesive patient care.
  • Maintain patient confidentiality and report any incidents or safety concerns.

Qualifications:
  • Current APRN state professional license (Adult/Geriatrics specialty preferred), DEA certificate, CPR certification.
  • Master's degree in Advanced Practice Nursing from an accredited school.
  • At least 2 years as a Nurse Practitioner, preferably in home health care.
  • Valid driver's license with reliable transportation.
  • Excellent communication, problem-solving, and organizational skills. Homecare Homebase knowledge is a plus.
