CHOR Youth & Family Services is seeking a compassionate Registered Nurse (RN) to provide nursing care and clinical support to children and adolescents receiving residential and behavioral health services in Reading, Pennsylvania.

What You'll Do
  • Provide compassionate nursing care to children and adolescents receiving residential and behavioral health services.
  • Complete nursing assessments and monitor physical, behavioral, and emotional health needs.
  • Administer medications and treatments in accordance with physician orders, nursing standards, and organizational policies.
  • Monitor medication effectiveness, side effects, changes, and potential health concerns.
  • Coordinate medical, psychiatric, dental, vision, and specialty healthcare services.
  • Communicate changes in health status, medication, treatment, and follow-up needs to the interdisciplinary treatment team.
  • Collaborate with psychiatrists, physicians, therapists, behavioral health professionals, direct care staff, families, and other members of the treatment team.
  • Help youth understand their health needs and develop healthy habits and greater awareness of their overall wellness.
  • Coordinate and follow up on medical appointments, laboratory work, specialty services, and other healthcare needs.
  • Support medication management, infection prevention, health and safety, and emergency response procedures.
  • Provide nursing guidance and health education to program staff.
  • Maintain accurate, timely, and confidential clinical documentation.
  • Participate in treatment planning and contribute the nursing perspective to each youth's overall plan of care.
  • Advocate for the health, safety, dignity, and well-being of every child and adolescent we serve.

Qualifications
  • Active Pennsylvania Registered Nurse (RN) license in good standing.

  • Ability to work independently while collaborating with an interdisciplinary treatment team.
  • Commitment to trauma-informed, compassionate, and youth-centered care.
  • Ability to meet all required background clearances, health requirements, and regulatory standards.
Experience in pediatrics, behavioral health, psychiatric nursing, mental health, residential treatment, adolescent health, community health, or human services is preferred but not required.
