What is a behavioral health discharge planner?

Behavioral Health Discharge Planners are professionals who coordinate care and services for patients transitioning from a behavioral health facility back into the community or another care setting. They assess patients' needs, develop discharge plans, and connect them with necessary resources such as outpatient treatment, housing, or support services. Their goal is to ensure a safe, effective, and supportive transition to prevent relapse or readmission. Discharge planners often work closely with medical staff, patients, families, and community agencies to create individualized plans.

What are some common challenges faced by behavioral health discharge planners when coordinating care after a patient's release?

Behavioral Health Discharge Planners often encounter challenges such as ensuring smooth communication between hospital staff, patients, families, and community providers. Navigating limited community resources or finding appropriate follow-up care can be particularly demanding, especially for patients with complex needs or limited support systems. Additionally, discharge planners must address issues related to patient compliance and engagement, while managing documentation and meeting regulatory requirements. Successfully overcoming these challenges requires strong organizational, interpersonal, and problem-solving skills.

What are the key skills and qualifications needed to thrive as a behavioral health discharge planner, and why are they important?

To thrive as a Behavioral Health Discharge Planner, you need a background in social work, psychology, or counseling, often with a relevant degree and licensure (such as LCSW or LPC). Familiarity with case management software, electronic medical records, and healthcare regulations is typically required. Excellent communication, problem-solving, and organizational skills are crucial for effective collaboration with patients, families, and multidisciplinary teams. These skills ensure smooth transitions from inpatient care to community support, reducing readmissions and promoting positive patient outcomes.

Job description

About the Role

We are seeking a high-performing Program Manager to oversee and optimize client experience and operational workflows across our behavioral health and recovery programs. This role is designed for someone who understands structured client journeys, cross-functional coordination, and outcome-driven program management. You will be responsible for ensuring programs run effectively from onboarding through completion, with a strong focus on engagement, retention, and measurable outcomes. While this is a healthcare environment, the mindset required is similar to program management in high-growth environments: ownership, process optimization, proactive problem-solving, and results-driven execution.

What You will Do
  • Oversee the full client lifecycle across multiple programs from onboarding through discharge
  • Drive engagement, retention, and program completion outcomes at a program level
  • Coordinate across clinical teams, case managers, and intake staff to ensure alignment
  • Identify gaps in processes and implement improvements to enhance client experience
  • Monitor program performance, track key metrics, and report on outcomes
  • Ensure consistent, structured workflows across all client touchpoints
  • Support escalation management for at-risk clients and complex cases
  • Lead initiatives to improve operational efficiency and scalability
  • Maintain accurate documentation and ensure compliance with internal processes
What We are Looking For

Ideal Experience:

  • 2–6+ years in Program Management, Customer Success, Operations, or Account Management
  • Strong experience managing workflows, processes, or multiple client/program tracks
  • Ability to coordinate across teams and manage competing priorities
  • Excellent communication, organization, and leadership skills
  • Data-driven mindset focused on engagement, retention, and outcomes
  • Experience in healthcare, behavioral health, or patient coordination is a plus (not required)
Highly Valued Backgrounds
  • Program Manager or Operations roles in tech or high-growth environments
  • Customer Success Manager (CSM) transitioning into program oversight
  • Account Management in subscription-based or service-driven industries
  • Healthcare coordination, case management, or patient services
Why This Role Is Different

This is not a traditional healthcare administrative role. This is a high-ownership position where you are responsible for how programs function as a whole. Your work directly impacts client engagement, program effectiveness, and long-term recovery outcomes. You will be building structure, improving systems, and shaping how care is delivered at scale.

Work Location

This is a hybrid role based in New Mexico. The position combines remote work with periodic in-person collaboration. Certain responsibilities, such as team coordination, program oversight, and operational alignment, may require onsite presence. Other responsibilities, including planning, reporting, and workflow management, may be performed remotely. Specific onsite expectations will be communicated based on program needs and team requirements.

What We Offer
  • Mission-driven work with measurable client impact
  • Opportunity to apply program management skills in healthcare
  • Collaborative and supportive team environment
  • Growth opportunities in operations, program leadership, and client success
  • Meaningful work where outcomes matter beyond metrics
