Job Title: Hospice Chaplain - East Bay County

Location: Fremont, CA

Job Summary:

We are seeking a compassionate and experienced Hospice Chaplain to provide spiritual and emotional support to patients, families, and caregivers in Contra Costa County. In this role, you will address the unique spiritual needs of individuals receiving end-of-life care, regardless of faith or belief system, ensuring dignity and comfort during this important time.

Key Responsibilities:

•Provide spiritual care and counseling to patients and their families, respecting diverse backgrounds, beliefs, and values.

•Collaborate with the interdisciplinary hospice team to develop and implement individualized care plans.

•Offer grief and bereavement support to families and caregivers.

•Conduct spiritual assessments and document care provided in accordance with hospice policies.

•Lead memorial services, blessings, or other ceremonies as requested by patients or families.

•Support hospice staff with spiritual guidance as needed.

•Build relationships with community faith leaders and organizations to provide additional resources for families.

Qualifications:

•Ordination or certification as a chaplain with endorsement from a recognized faith community or chaplaincy organization.

•Minimum of 2 years of experience in hospice or healthcare.

•Strong understanding of end-of-life care and grief processes.

•Ability to provide non-denominational support to individuals of all faiths or no faith.

•Excellent communication and interpersonal skills.

•Valid driver’s license and reliable transportation for travel within Contra Costa County.
